Your parent or guardian must complete a hard-copy of this form if you are under 18 years old. The following form must be completed and handed to the Volunteer Manager during the in-person interview.
I hereby consent to the participation of my daughter/son, in the KACCH & BACCH Volunteer Programme. I understand that my son/daughter must fulfill the volunteer programme requirements in order to receive a certificate for his/her volunteer participation.
I also give permission to use any photographs that are taken of my daughter/son, while s/he is volunteering with KACCH & BACCH, for the use of publicity in promoting KACCH & BACCH activities without limitation or reservation.
